Coordination of Care Community - December 1st Meeting (Presentation from Orion Health) 9 years 6 months ago #891

As noted, our presentation from Orion Health will be the focus of next week's monthly meeting (Tuesday December 1st, 11AM). Here are some updated details.

Title: Information and care coordination in support of patient transitions across continuum of care

The success or failure of care coordination across care delivery settings depends on access to comprehensive and accurate longitudinal patient record. Orion Health has deployed electronic health records systems in seven Canadian provinces, which serve as the basis for care coordination among care teams and clinicians.

The webinar will examine two care coordination case studies: a care coordination tool deployed in Ontario, and an e-Referral deployment in Alberta. Both projects build on the base architecture of the EHR including healthcare service bus; clinical data repository and clinical portal.

About our Speakers

The December 1st webinar will be jointly presented by Dr. Chris Hobson, Chief Medical Officer at Orion Health, and Susan Anderson, Managing Director for Canada at Orion Health.

Dr. Hobson has dedicated his work to developing and applying health information technology to the task of improving healthcare system outcomes across the triple aim – cost, quality and the patient experience of care. His experience in Healthcare IT has ranged from business and clinical strategy through product design and development, sales and marketing, implementation project management and business. Prior to his work with Orion Health, Dr. Hobson served as Clinical Program Director for the Integrated Care and IT initiatives for the Counties Manukau District Health Board, and practiced as a family physician in Auckland, New Zealand.

Susan Anderson recently joined Orion Health taking on the role of Managing Director for the Canadian Business. Prior to working at Orion Health, she served as Assistant Deputy Minister for the Health Information Technology and Systems Division and CIO for Alberta Health. There she played a pivotal role in the expansion of EHR data sources, deployment to users, integration for physicians and pharmacists, and introduced public access to EHRs. Susan has been a leader in Canada’s healthcare informatics sector for over 20 years and serves on the Board of Directors for COACH, Canada’s Health Informatics Association.
