In late fall we launched a pilot care planning initiative in Alberta called 'Patients Collaborating with Teams' (PaCT). It was recognized early in the design process that a care plan template would be a key tool for patients and teams. A small team of individuals from a number of partnering organizations came together to agree on a set of principles, work with stakeholders, and to scan the environment for a care planning template that could be tested by the clinics participating in the pilot.
The draft care planning template, with an accompanying document that outlines the background, methods, and design principles, are available on the webpage for the PaCT Initiative: http://www.topalbertadoctors.org/pact/toolsresources/ We would be keen to participate in the working group to learn from others and share our learnings as we get feedback from patients and teams on the draft we have. |
